Rush appointment fee — starting at $25

After-hours fee — starting at $35

Weekend/holiday fee — starting at $35

Hospital / care facility surcharge — starting at $35

Scanbacks / faxbacks — starting at $18

Printing — $0.20 per page (black & white if required)

Mobile Travel Distance Fees

0–20 miles → $30 travel fee

21–30 miles → $40 travel fee

31–40 miles → $50 travel fee
